Do I get a tax receipt?

When the receiving charity provides one, yes, and your Loader passes it along. Charities issue receipts at their own discretion based on what they accept.

What if my mattress is not donation quality?

It moves to the next best path automatically, recycling where facilities exist, responsible disposal as the last resort, at the same guaranteed price.
